Donepezil

Anticholinesterase, anti-dementia agent

Also sold as Aricept, Aricept ODT, Adlarity.

Findings for this medication

Research about this medication, graded for how strongly each finding can be attributed to it. Nothing here describes your own levels.

Donepezil has no classical depletion mechanism. It is not a chelator, does not induce hepatic enzymes, is not renally wasted, and its own label reports little or no direct inhibition of the CYP enzymes checked. There is no documented food, mineral, or antacid interaction that alters its absorption, and food does not change how much is absorbed. The real signals in this dossier come almost entirely from two other places: donepezil's own peripheral cholinergic (vagotonic) pharmacology, which can slow the heart and, in a bounded subgroup, cause bradycardia or conduction block; and a genuinely interesting, drug-specific line of mechanistic and small clinical evidence that donepezil itself, apart from any nutrient, appears to support mitochondrial function and redox status in treated patients relative to untreated Alzheimer's disease, most plausibly because it is reducing the amyloid-beta burden that itself damages mitochondria.

Anticholinesterase, anti-dementia agent · ATC N06DA02 · also sold as Aricept and Aricept ODT and Adlarity and Namzaric (with memantine) · reviewed 2026-09-29. This describes research about the medication. It does not say anything about your own levels or health.

How to read the evidence grades

These colours describe how strongly a finding can be attributed to the medication in the research. They are not a severity score and they say nothing about your own health.

Grades describe how strongly a finding can be attributed to Donepezil in the research — not how common it is, how serious it would be, and not anything about you personally.

Red — Severe
Two or more independent Tier A sources, this drug in the cohort, the nutrient directly measured, consistent direction, clinically meaningful magnitude, a named mechanism, no substantial contradicting Tier A evidence.
Orange — Moderate
At least one Tier A source with this drug in the cohort showing a real but bounded deficit: an exposure window, a subgroup, low frequency, or a functional rather than circulating deficit.
Yellow — Low
Mechanistic plausibility only (Tier B), or class extrapolation (Tier C) with no drug-specific human status data; also used for context flags and interactions running the other direction.
Not graded — disclosed evidence gap
Nothing has been measured here, so this finding cannot be graded. That does not establish either safety or harm, and it is not the same as a White result.Source definition: Not gradeable under the rules above because nothing has been measured. Published as a documented gap, never as reassurance that there is no effect.

What the research found

Global nutritional status

Orange — Moderate

See study details

Donepezil's cholinergic GI effects measurably reduce food intake in a defined subgroup, though the largest controlled follow-up found no significant weight or BMI change at 6 months.

Full detail and limits

What the grade means here: At least one Tier A source with this drug in the cohort showing a real but bounded deficit: an exposure window, a subgroup, low frequency, or a functional rather than circulating deficit. orange

Limits on what this shows

  • Key null finding: The 6-month controlled study found no significant weight or BMI change despite the measured decline in food intake, meaning this is currently a functional intake finding, not a confirmed weight-loss or malnutrition outcome, at the population level.
  • At-risk subgroups: A separate case series of 8 dementia patients on cholinesterase inhibitors (including donepezil) documented involuntary weight loss of 14 to 22 kg over 18 months in individuals who were unusually GI-intolerant, illustrating the tail risk in susceptible patients even though it is not the population average.

Regulatory status

In the pivotal severe-Alzheimer's trial, anorexia occurred in 8% of donepezil patients versus 4% on placebo, vomiting in 8% versus 4%, diarrhea in 10% versus 4%, and nausea in 6% versus 2%, all at twice the placebo rate or higher.

Population evidence

A prospective 6-month observational study of 116 newly diagnosed dementia patients compared donepezil, rivastigmine, and galantamine as independent treatment arms using the Mini Nutritional Assessment Short Form. Food intake worsened across all three drugs (kappa 0.377 agreement across raters), with donepezil-treated patients showing significantly greater decline in food intake than rivastigmine-treated patients (p<0.05). Weight and BMI did not change significantly in any arm over 6 months (p>0.05).

Mechanism

Named: peripheral cholinergic stimulation of the GI tract produces nausea, vomiting, diarrhea, and anorexia (mechanism class H, changed intake), most pronounced during dose initiation and titration.

Counter-evidence held on record

The 6-month MNA-based study is the strongest available population evidence and it did not find significant weight or BMI change from donepezil, which tempers the case-series signal considerably.

Homocysteine / vitamin B12 status

Not graded — disclosed evidence gap

See study details

No isolatable donepezil-specific evidence and no circulating consumer claim exist for this marker.

Full detail and limits

What the grade means here: Nothing has been measured here, so this finding cannot be graded. That does not establish either safety or harm, and it is not the same as a White result. ungraded

Limits on what this shows

  • Why no grade: Both conditions for ungraded are met: no Tier B or C evidence isolates a donepezil-specific mechanism or effect on homocysteine or B12 (the only human data found is confounded by three co-administered treatments), and no circulating consumer claim exists for this review to rebut as white.

Channels tried

Europe PMC and OpenAlex searches combining donepezil with homocysteine, vitamin B12, and folate; PubMed search-listing attempts (blocked by bot-verification, see access note); Drugs.com interaction checker for donepezil and vitamin B12; general consumer nutrient-depletion phrasing searches.

Outcome

One retrospective study of 124 Alzheimer's patients found reduced serum homocysteine and improved sleep quality in a group given folic acid, vitamin B12, memantine, and donepezil together versus a standard-care control group, but the four co-administered interventions cannot be separated, so the homocysteine change cannot be attributed to donepezil specifically. Drugs.com reports no known interaction between donepezil and vitamin B12. No mechanism in the Step 2 screen (donepezil is not a folate antagonist, does not affect intrinsic factor or B12 absorption, and does not induce hepatic enzymes) supports a plausible pathway.

Genuine gap vs. tooling limit

This reads as a genuine evidence gap rather than a tooling limitation. Low B12 is well studied as a cause of reversible cognitive impairment that mimics Alzheimer's disease, which is why clinicians check it before diagnosis, but that clinical practice is not evidence that donepezil itself changes B12 or homocysteine status, and no study attempting to isolate that question was found.

The five body-system areas

Each finding is counted once, in a single area. Where a study is mentioned in a second area it is context only.

Mitochondrial Burden

Each finding below is counted once, in this area only.

Mitochondrial burden

Yellow — Low

Direction: reverse direction (as labelled in the source)

Donepezil-specific animal and cell evidence shows the drug protects mitochondrial function against amyloid-beta toxicity; one human observational study found preserved skeletal-muscle mitochondrial respiration in treated versus untreated mild cognitive impairment, but confounding by indication is not excluded.

Full detail and limits

What the grade means here: Mechanistic plausibility only (Tier B), or class extrapolation (Tier C) with no drug-specific human status data; also used for context flags and interactions running the other direction. yellow, reverse direction

Limits on what this shows

  • Key limitation: The human study is observational, not randomized; treatment assignment was not controlled, so confounding by indication (healthier or more treatment-adherent patients being the ones on donepezil) cannot be excluded per the standard applied throughout this series.

Mechanism

Named, drug-specific, Tier B: in transgenic amyloid-precursor-protein mice and in vitro systems, donepezil reduced mitochondrial accumulation of amyloid-beta, modulated the mitochondrial permeability transition pore, preserved glutathione, and restored mitochondrial energy production capacity, independent of acetylcholinesterase inhibition.

Population evidence

A human study comparing 24 cognitively healthy older adults and 26 adults with mild cognitive impairment (some donepezil-treated, some not) found untreated MCI participants had reduced skeletal-muscle mitochondrial respiratory capacity relative to both healthy controls and donepezil-treated MCI participants; donepezil treatment was also associated with altered expression of over 600 skeletal-muscle genes.

Disease-vs-drug note

Mitochondrial dysfunction is a well-established, disease-intrinsic feature of Alzheimer's disease and MCI. The finding here is specifically that donepezil-treated patients look less impaired than untreated patients with the same disease, which supports a drug effect but does not establish it with the certainty a randomized trial would.

Oxidative Stress

Each finding below is counted once, in this area only.

Oxidative stress

Orange — Moderate

Direction: reverse direction (as labelled in the source)

In a small human comparative study, donepezil-treated Alzheimer's patients showed better antioxidant and lower oxidative-damage markers than untreated Alzheimer's patients, a protective association rather than a depletion.

Full detail and limits

What the grade means here: At least one Tier A source with this drug in the cohort showing a real but bounded deficit: an exposure window, a subgroup, low frequency, or a functional rather than circulating deficit. orange, reverse direction

Limits on what this shows

  • Key limitation: Single-site, comparative (not randomized or placebo-controlled), moderate sample size (n=52 patients). Confounding by indication cannot be excluded: patients selected for or tolerating donepezil treatment may differ systematically from untreated patients in disease severity, comorbidity, or frailty.

Population evidence

A comparative human study of 52 Alzheimer's patients (23 on donepezil, remainder untreated) and 32 healthy controls measured protein carbonyl (PCO), advanced protein oxidation products, ischemia-modified albumin, ferric reducing antioxidant power (FRAP), total and protein thiol, and kynurenine metabolites. Untreated Alzheimer's patients had significantly higher PCO than both the donepezil-treated group and healthy controls; the treated group showed higher FRAP, total thiol, and protein thiol, and lower kynurenine and N-formyl kynurenine, than the untreated group.

Mechanism

Not a direct antioxidant chemistry of donepezil itself; the most plausible pathway, supported by the mitochondrial entries below, is that reducing amyloid-beta-driven mitochondrial and oxidative injury secondarily improves systemic redox markers.

Disease-vs-drug note

Alzheimer's disease itself is independently associated with substantial oxidative stress in the broader literature; this entry reflects a comparison between treated and untreated disease, not a comparison to nutrient-replete healthy status, and is graded as a bounded, single-study finding for that reason.

Counter-evidence held on record

This is not evidence that donepezil requires antioxidant supplementation, nor that withholding donepezil protects redox status. Direction runs opposite to a depletion story: less treatment is associated with worse markers in this dataset, not better.

Copper and Iron Balance

Copper and iron are assessed separately. Nothing here is counted twice.

Copper

Yellow — Low

See study details

Serum copper runs higher in Alzheimer's disease patients than in healthy controls as a feature of the disease; no study in this review measured whether donepezil itself changes copper status.

Full detail and limits

What the grade means here: Mechanistic plausibility only (Tier B), or class extrapolation (Tier C) with no drug-specific human status data; also used for context flags and interactions running the other direction. yellow, disease-linked context flag

Population evidence

A meta-analysis of 44 case-control studies found serum copper significantly higher in Alzheimer's disease patients than controls (mean difference 9.27, 95% CI 5.02-13.52, p<0.0001). This meta-analysis did not evaluate treatment status or any specific drug.

Drug-specific evidence

None located. A rat study using copper sulfate to induce Alzheimer's-like neurodegeneration tested donepezil as a treatment for the resulting cognitive and inflammatory endpoints (acetylcholine, BACE1, phosphorylated tau, apoptotic markers), but did not report donepezil's effect on copper levels themselves, so it does not support a copper-specific claim for this drug.

Mechanism

None identified for donepezil directly. No chelation, transporter, or renal-handling mechanism connects donepezil to copper in the pharmacokinetic or mechanism screen.

Disease-vs-drug note

Per the confounding-by-indication standard applied throughout this series, an elevated copper finding present in Alzheimer's disease broadly cannot be attributed to donepezil without a study comparing treated to untreated or to pre-treatment baseline, which does not exist in the literature reviewed here.

Iron

Yellow — Low

See study details

Serum iron runs lower in Alzheimer's disease patients than in healthy controls as a feature of the disease; no study in this review measured whether donepezil itself changes iron status.

Full detail and limits

What the grade means here: Mechanistic plausibility only (Tier B), or class extrapolation (Tier C) with no drug-specific human status data; also used for context flags and interactions running the other direction. yellow, disease-linked context flag

Population evidence

The same 44-study case-control meta-analysis found serum iron lower in Alzheimer's disease patients than controls after sensitivity exclusion of two outlying studies (mean difference -13.01, 95% CI -20.75 to -5.27, p=0.001). Serum zinc was also lower (mean difference -6.12, 95% CI -9.55 to -2.69, p=0.0005). Neither analysis evaluated treatment status.

Drug-specific evidence

A rat study of nanocurcumin and donepezil in an aluminum chloride/D-galactose Alzheimer's model tested seven groups, including a donepezil-alone arm, and reported that nanocurcumin alone or combined with donepezil improved iron profile and ferroptosis markers; the abstract and available data do not isolate an iron-specific effect for the donepezil-alone arm apart from the nanocurcumin-containing arms, so this cannot support a donepezil-specific iron claim.

Mechanism

None identified for donepezil directly in the pharmacokinetic or mechanism screen.

Disease-vs-drug note

As with copper, the lower-iron finding is a disease association, not a treatment effect, per the same confounding-by-indication standard.

Gut Microbiome Risk

Each finding below is counted once, in this area only.

Gut microbiome risk

Yellow — Low

See study details

Donepezil altered gut microbial composition and improved intestinal barrier integrity in two independent amyloid-beta mouse models; direction trends toward restoration rather than harm, but nothing has been studied in humans.

Full detail and limits

What the grade means here: Mechanistic plausibility only (Tier B), or class extrapolation (Tier C) with no drug-specific human status data; also used for context flags and interactions running the other direction. yellow

Limits on what this shows

  • Key limitation: All three sources are rodent studies; no human gut-microbiome data for donepezil were located in this review.

Mechanism

Not fully characterized; proposed pathway is the cholinergic anti-inflammatory reflex (vagal cholinergic signaling modulating gut inflammation and barrier function) rather than direct antimicrobial action.

Population evidence

In female APP/PS1 transgenic mice given donepezil 3 mg/kg/day orally for 24 weeks, treatment increased Akkermansia (about 54% higher), Clostridium, Odoribacter, and Ruminiclostridium, decreased Bacteroides, Helicobacter, Mucispirillum, Lactobacillus, and Prevotellaceae, restored intestinal barrier integrity, and reduced colonic inflammation relative to untreated APP/PS1 mice. A second, independent amyloid-beta-induced mouse pilot model also reported donepezil-associated shifts in gut microbiota and gut metabolites alongside brain metabolite changes. A third study found donepezil ameliorated gut barrier disruption in a doxorubicin-treated rat model unrelated to Alzheimer's disease, supporting a general barrier-protective mechanism rather than one specific to amyloid pathology.

Counter-evidence held on record

The available animal direction is toward barrier restoration and reduced inflammation, not toward a harmful microbiome disruption. This entry should not be read as a caution against donepezil on gut grounds; it is graded yellow because the evidence tier (animal-only) caps it there regardless of direction.

Oxygen and ATP Output

Oxygen delivery and ATP production are assessed separately. Nothing here is counted twice.

Oxygen delivery/utilization

Orange — Moderate

See study details

Donepezil's own cholinergic pharmacology can slow the heart and, in a bounded subgroup, impair perfusion via bradycardia or heart block; this is a cardiac safety signal, not a nutrient deficiency.

Full detail and limits

What the grade means here: At least one Tier A source with this drug in the cohort showing a real but bounded deficit: an exposure window, a subgroup, low frequency, or a functional rather than circulating deficit. orange

Limits on what this shows

  • Key null finding: A donepezil-specific systematic review and meta-analysis of 60 randomized trials (over 12,000 participants) found no association between donepezil and fatal, QT-related arrhythmia or associated death. A separate pooled meta-analysis across donepezil, rivastigmine, and galantamine (25 studies, n=258,388) found elevated relative risk for both bradycardia (RR 1.23, 95% CI 1.14-1.33) and syncope (RR 1.40, 95% CI 1.20-1.64), but did not stratify results by individual drug.
  • At-risk subgroups: Patients with pre-existing sinus node disease, AV conduction disease, or on other rate-slowing agents; risk is concentrated at initiation and dose escalation (5 mg to 10 mg to 23 mg).

Regulatory status

The FDA label states directly that cholinesterase inhibitors "may have vagotonic effects on the sinoatrial and atrioventricular nodes," manifesting as bradycardia or heart block, with or without pre-existing conduction abnormality.

Population evidence

A case-time-control study of Ontario residents aged 67 and older (161 cases hospitalized for bradycardia, 466 matched controls; donepezil was the dominant drug, 73% of cases and 63% of controls) found recent initiation of a cholinesterase inhibitor associated with more than double the risk of hospitalization for bradycardia (adjusted OR 2.13, 95% CI 1.29-3.51). Pacemaker insertion occurred in 11% of hospitalized cases.

Mechanism

Named: peripheral acetylcholinesterase inhibition increases vagal tone at the sinoatrial and atrioventricular nodes, slowing conduction. This is an off-target systemic extension of the drug's cholinergic action, not its intended CNS therapeutic mechanism.

Counter-evidence held on record

This is a cardiac conduction adverse effect, not a nutrient or mineral deficiency, and nothing about it is corrected by any supplement. There is no FDA label language calling for correcting potassium or magnesium before use, and none should be inferred. The action is clinical monitoring of heart rate and syncope symptoms, particularly at dose changes, not a nutritional intervention. Do not read this entry as evidence that donepezil depletes a mineral involved in cardiac conduction. No such mechanism is documented.

ATP production

Yellow — Low

Direction: reverse direction (as labelled in the source)

In cell and ex vivo hippocampal models, donepezil dose-dependently increased mitochondrial biogenesis and ATP levels through AMPK/PGC-1α signaling; no human ATP measurement exists for this drug.

Full detail and limits

What the grade means here: Mechanistic plausibility only (Tier B), or class extrapolation (Tier C) with no drug-specific human status data; also used for context flags and interactions running the other direction. yellow, reverse direction

Limits on what this shows

  • Key limitation: All supporting evidence is cell-based, ex vivo, or animal; no human study in this review measured ATP production or mitochondrial biogenesis markers directly in donepezil-treated patients.

Mechanism

Named, drug-specific, Tier B: in neuroblastoma cells, primary hippocampal neurons, and organotypic hippocampal tissue from adult mice, donepezil dose-dependently increased mitochondrial biogenesis and ATP levels along with PGC-1α and NRF-1 expression; blocking AMPK eliminated the effect, identifying AMPK as the mediator. A separate transgenic-mouse and in vitro study reported donepezil restored mitochondrial energy production capacity reduced by amyloid-beta toxicity.

How the body handles this medication

Absorption
Oral bioavailability about 100%, peak plasma concentration at 3 to 4 hours. Food does not change the rate or extent of absorption; no antacid or mineral interaction is documented.FDA label [1]
Protein binding
About 96% bound to plasma proteins (about 75% albumin, about 21% alpha-1-acid glycoprotein), leaving little free fraction to interact with circulating nutrients directly.FDA label [1]
Metabolism
Metabolized via hepatic CYP2D6 and CYP3A4 plus glucuronidation. In vitro data show little or no direct inhibition of CYP2B6, CYP2C8, or CYP2C19 by donepezil, and the label does not describe donepezil as an enzyme inducer of any CYP.FDA label [1]
Elimination
Elimination half-life about 70 hours. About 17% of a dose is recovered unchanged in urine; roughly 57% of total radioactivity is recovered in urine and 15% in feces over time, so donepezil is not renally wasted intact.FDA label [1]
Organ impairment
Clearance did not differ in a small study of moderate to severe renal impairment. In hepatic impairment, clearance decreased by about 20%, so drug (not nutrient) exposure rises modestly, which is relevant to dosing rather than to nutrient status.FDA label [1]
Cardiac mechanism
The label states cholinesterase inhibitors "may have vagotonic effects on the sinoatrial and atrioventricular nodes," which "may manifest as bradycardia or heart block" with or without pre-existing conduction disease. This is the mechanistic basis for the oxygen delivery/utilization entry below.FDA label [1]
